CHAPTER EVENT RELEASE FORM FOR ADULTS

 

Adult Non-Member or Expired Members needs to fill out this form. (age 18 and over)

CHAPTER EVENT RELEASE FORM FOR MINORS

 

 

A Minor needs to have Parental or Guardian approval to be on the ride.
By signing the CHAPTER EVENT RELEASE FORM FOR MINORS
the Parent or Guardian gives that consent. (under age 18)

MINOR'S ASSUMPTION OF RISK ACKNOWLEDGEMENT

 

The Minor also needs to acknowledge the risk of going on the ride themselves.
In addition to the CHAPTER EVENT RELEASE FORM FOR MINORS
being signed by the Parent or Guardian, the Minor must sign
the MINOR'S ASSUMPTION OF RISK ACKNOWLEDGMENT form. (age 12 and over)

 

Note: If someone shows up with a Minor and they are not the Parent or Guardian,
they must have the form signed by the actual Parent or Guardian to be valid.
(Example: They brought a friend who is a Minor,
they are the Grandparent not the Guardian legally, etc.)

 

CHAPTER INJURY REPORT

This form needs to be filled out as soon as able in the occurrence
of an incident or accident at a Chapter event or ride.


COVER LETTER FOR CHAPTER MEMBERSHIP ENROLLMENT FORM AND RELEASE

This letter is to be given to the Member along with the
CHAPTER MEMBERSHIP ENROLLMENT FORM AND RELEASE.

CHAPTER MEMBERSHIP ENROLLMENT FORM AND RELEASE

This form is to be filled out by New Chapter Members and Renewing Members.
They only have to submit this form once yearly. (Don't forget to give the Member the COVER LETTER also)
